The School Board of Pinellas County requires that all athletes complete a battery of forms, to be turned in prior to the start of Band Camp and kept on file for the upcoming school year. This packet consists of notarized liability forms, which outline the inherent risks of heat related activities, proof that school insurance has been purchased, and a physical form, clearing each student to participate in the Band Program and signed by a physician. In addition, all permission slips for the season will be included, along with volunteer forms, drivers forms, and more.
In addition to receiving, filling out, and returning your forms packets, the following can also be accomplished at Forms Night:

- Choose and Pay For Band Camp Meals
- Receive Required Physicals From A Licensed Doctor
- Turn in Required Volunteer Forms and Drivers Forms
- Receive Charms (financial tracking software) Tutorials and Login Help
- Attend Band Camp 101 and New Parent Orientation Meetings

There are typically a minimum of two Forms Nights each year – one in early June and the other in early July. Dates for the 2017-18 school year are as follows:
Saturday, June 3, 2017 @ 7:45pm
Tuesday, June 27, 2017 @ 7pm
Forms Nights are held in the SHS Auditorium and are strongly recommended for ALL Band Families. If you were unable to attend one of the two Forms Nights this year, please visit the Download Forms page and print, fill out, and return the required forms to the Blue Box in an envelope addressed to Kim Bernstein prior to the start of Band Camp.
